    Since you have taken time to have tests to rule out anything else that could be causing the pain-have you thought of going to another dentist for another opinion? If your in that much pain after the procedure was done and it's been a good length of time I'd definitely go to another dentist. I switched from my HMO coverage back to Blue Cross so I could go back to my original dentist since the one I had seen in the HMO caused me to lose 2 teeth that I did not have to lose. You have everything to gain (pain relief) by seeing another dentist. Before making an appointment ask the (new) dentist office if you need to bring a copy of your x-rays with. Your current dentist will let you borrow the x-rays (I've done it) to take to another doctor. If you feel uncomfortable telling your current dentist your seeing another dentist then just tell them your going to an eye, ear, nose & throat specialist instead. Your main objective is to be rid of the pain. You could possible have an infection which if left untreated could get into your bloodstream and cause you other problems.
