    Reg W7 form
    Hi,

    Is it required that the W7 form itself be also notarized or is it that only the passport copies be notarized?

    Only the passport copies need be notarized.

    The Form W-7, however, must be signed by the applicant if the applicant is an adult. If the applicant is a child, the parent must sign.
