    #19 Tooth ache
    I had a root canal on #19 in Dec 2008. This is after a Crown was placed on it in 2007 due to an old filing. The tooth has some pins due to a break years prior and a filling at age 13 which was around 1970.

    The tooth started to ache off and on around 2 weeks ago. Went back to the Endodontist and he did X-rays, stated the root canal looked fine. Call if it gets worse. Went to my general dentist to check the other teeth, all good, nothing wrong, but stated, the tooth could have a crack under the crown the X-rays do not show and maybe consider extraction and implant.
    Is that the only solution? The general dentist did say, you can have the root canal done again, but no guarantees it would fix if it is a crack or fracture of the tooth. No way to tell, and then it gets down to Dollars.. pay for a 2nd root canal or remove the tooth and get an implant. Is this the only solution or should I go to a Oral Surgeon for an opinion.. but time is short as it is getting worse..

    I went through a similar deal, choice was extraction, extraction and implant or root canal and pray it fixes it.

    Well I did the root canal, I was advised it wouldn't work and low and behold... it didn't.
    So I extracted and implanted, cost some $$$$ but it was worth it.

    Has the dentist checked the bite? You could be banging the crowned tooth by clenching or grinding on it. A simple fix if that's the problem.
    It's true that a crack is almost impossible to find unless the crown is removed, the filling material in the canal is removed and the dentist has a good pair of loupes that he wears or some kind of magnification.
    If I remember correctly that in the US tooth 1.9 is at the top front... when you close your mouth slowly do your lower teeth touch that tooth first?

    #19 is on the bottom, left, I had my wisdom tooth removed so it is today, # 2 from the back, left side.. I guess it would be # 3 if I had my wisdom tooth.

    Last week I went to the Endodontist (root canal specialist) for x-rays and such.. he saw no infection in the X-rays and the rootcanal looked good he stated.

    I went to the General Dentist 4 days later to check all the other teeth and that one as well.. said all is good on the other teeth and the root canal in the X-rays looked good.

    So no news is bad news.

    The General Dentist found no signs of grinding of the teeth.. but did said I clentch my jaw (Stress) in the job and life I guess. He said it may help to wear a mouth guard, but did not think this pain since # 19 is a bit tender to tapping on it.. that a mouth guard would fix it.

    So back to the Endodontist next week.. If I chew gum, the tooth gets a bit alarmed, but I think it does not get really bad as I had a root canal already and since most of the nerves are removed already.. it has limited nerves to ache.. but this is from down below.. jaw area.. so maybe an infection.. or maybe he missed another hidden nerve. The General dentist suggested removal of the crown by the endodontist to check the tooth and re-do the root canal and then put the crown back on afterwards and as always, go back to the General dentist to close the crown. Money for all I guess.

    I have been corrected (more than once;)). Sorry folks! (No idea why the US has to number their teeth differently than we do here up north.)

    If you are clenching during the day you are more than likely doing the same at night. My feeling is that a night guard is a less expensive way to go to try to solve the problem. I have not seen many crowns come off a tooth without having to be cut off unless a temporary cement was used.

    If you sit at a computer all day watch your posture. Computer work often puts people in to a forward head posture which is condusive to clenching. Do you have any symptoms such as headaches in the temple area when you wake in the morning? Ear aches? Tooth that gets hypersensitive once in awhile and then settles down? Clicking/pain in the jaw joint? If yes to any of these, definitely a night guard is the way to go.

    Oh if you have a root canaled tooth with PINS that is a recipe for a crack! Sometimes the crack is hard to see on the x-ray but I usually try to probe around the tooth to look for a deep pocket... I had a case like this where a guy had a gold crown and a root canal... had a deep pocket... I extracted it and there was a crack with a little ball of infected tissue (granuloma). If its bugging you id get the implant
