You need to find out who the administartor of the 401(k) plan is. When you first registered for the plan you received information on this, and ever since you signed up you should have been receiving quarterly statements from them. So try to dig up an old statement - it will have the contact information on it. If you've lost all your records and aren't currently receiving quarterlystatements, I'm guessing it's because you have moved and didn't notify them to keep your address up to date. In this case it's easiest to just call the HR department at former employer and ask them who to contact. Or if you have any old acquaintances who have worked there, call one of them - if they have maintained their own accounts they should be able to tell you who to contact.
