I paid off my house in December of 2006. I never received a title or deed, just some paperwork stating the loan was paid in full. What do I do next? And can I add my son's name to anything to insure he gets my home if I die?

Our deed should have been filed when you bought the house at the courthouse along with the mortgage, You verify that the mortgage is shown as paid in full at the court house.
You can add the son if you want but you can also merely list him on a will, put the home in a trust and other thngs depending on many issues.
I don't know your age or health condition but if you put his name on the house, you may possibly be considered as having given away assets and can possibly be penalized for 5 years if you should require long term care and especially Medicaid assistance. The will is the best option I'm my opinion but I am not an elder law attorney.
One other thing you should consider if you put his name on title is to make yourselves "joint tenants". This means that when one of you passes away, the house goes to the other alone, with no probate issues.
