What if Canada immigration found out that an immigrant submit fraud medical health documents on applying to Canada? Are there any punishments? Or his residency status will be cancel?

It's fraud and the application would be cancelled if it's not been accepted yet.

Also you have to clarify your status, a permanent resident has been given the right to live in Canada, but they are not a citizen, a permanent resident can be forced to leave the country if they commit a serious crime or violate any immigration laws (like forged documents in this case).

So yes they can make you leave if they find out the documents are forged OR they could let you stay and impose fines of about $10,000 per false document.
