I completed a degree in USA and worked 6 months on OPT in 2005. I then left for my home country. I am now back in US. Can I apply for OPT extension, i.e. to utilize the remaining 6 months?

Under the new Extension rules u need to be employed by an employer who is enrolled in E-Verify.
