I was recently served a summons for credit card debt and plan to respond myself. Is there any type of document that is already formatted that I can put my own information on to help me?

You have to file a Notice of Intend to Defend, sometime it is attached to the document you received.

Once you done this send a Discovery request to the other party, to produce document, accounting of the account, original contract you signed, etc.
