Call the concrete company. Give them the size SQ. FT. and thichness you need. They can calculate the price of the materials from there. For the labor call around and get estimates. All areas price differently.

I'm not clear on what you need an estimate for. If you haven't done something like this before and you are planning to do it yourself... please know this... if you make a mistake on the slope (low point - high point - not in the right place), grading not "even" under the new slab (will promote cracking) or any other oversight that may be made... removal of the new concrete, is a job that can be as difficult and expensive as the original installation was.

I would suggest getting an estimate (usually free) from a licensed contractor and discuss the specific requirements of your site with him/her. You don't have to hire them but it'll be helpful to get some input.

Oh yeah... if you use 2X4 forms and your proposed new slab is expected to be about 3-1/2 to 4" deep... figure 1 cubic yard of concrete for each 80 square feet of floor space. Good luck!!

4" concrete patios can usually be done for about $4.00/sf including material and labor. Material only would cost about $2.00/sf.
