Need to know what type of fan this model number is. I need a switch for the fan but there is nothing else on the fan

Ceiling Fan? Kitchen Exhaust Fan? Bathroom Exhaust Fan? Workshop Exhaust Fan?

I did a quick Bing search and this model; number came back as Hunter ceiling fan.
That said, head to Home Depot and pick up one of their Universal Fan switches.
A word of warning, before you pull the switch out, make sure you mark the exact wiring positions or at the very least take a picture of the wire locations.
