View Full Version : ITIN Application - Medical Record for Dependent

Hi Experts,


According to W7 instructions I understand that I have to attach a US medical record for the proof of US residency for my Kid who is 1 year old to apply ITIN and claim as dependent in my Tax return . I have a medical/clinical summary for a visit from the hospital which has everything required by IRS except the Docto(or)hospital phone number. Based on IRS instruction if medical record is missing any required information then medical record should be - accompanied by a dated letter providing the required information on official letterhead from the federal authority, physician, hospital, or clinic that administered the latest care of the child


Any idea on
1) How to request a medical record from a Hospital for ITIN purpose
2) What content (required information)/ format should be there in the letterhead

Call the hospital and ask for administration, and hope they have a clue. This is a new requirement for ITIN applications, so a lot of hospitals will probably be unfamiliar with how to respond.

Thanks for the reply. My daughters passport has a valid date of entry to USA (official seal from port of entry at airport with the date written) . Do we still need to attach a medical record for US residence proof? New requirement is little confusing. Thanks for the help.

You would think the entry stamp would be enough, but the IRS ITIN Center has specifically requested some type of extra documentation to prove that the ITIN applicant is located in the United States.

Any idea on the extra documentation? Or should I submit medical record along with passport to be on safer side?

I would submit the medical records, just to be safe.
