I just notice that after I submitted my FBAR for the 2014 tax year, I did not include the tax year next to my name. I should contact them and let them know?

Scroll down to page #2 of FinCen Form 114. If you entered the tax year on the top of the page, you are good.

If not, re-submit it as an amended FBAR.
