I believe that the daughter of a former girlfriend may be my child. I have never been told that I had a child. I have had no contact with the ex in over 10 years, and the child is now 11. Could my name be on a birth cert without me knowing it? There has never been a request for CS. What should I do?

Consult a lawyer. But like the Con said it all depends on what you want. I personal think it should be checked out. All kids have the right to know both parents. Although it seems strange that you would not have ever been told, you hear about these things. But you will have to start some court stuff.
It's stuff you can do yourself, but a attorney can advise you for a small consult fee. He will do all the work for you but that will cost more, but it sounds like you will need one if you want any visitation.
