Does anyone know or have experience with the trams abdominal cerclage?

Know of it, but have not had one myself. Relatively new procedure... few years, and not done very often. Have you had a cervical cerclage in the past?

Yes, I had a mcdonald cerclage placed at 18 weeks with my son. I had him at 24 weeks.

In some cases it may be the best, or only, option of avoiding preterm birth. Again, not done often, although gaining popularity in the last few years when a standard cerclage is not successful or possible.
Would it be possible for you to have another standard cerclage? Having one birth at 24 weeks does not automatically mean another would come early as well. It would depend on if a reason was determined for the preterm birth and/or if it would not be possible for your cervix after the previous birth.

When I found out I was pg with my son the dr said we will monitor because I had previous miscarrage at 16 weeks. She said miscarriage arntbuncommon but 16 weeks is pretty far along. So with my son we pretty much watched my cervix shrink and waited for the funneling to happen. This time we know that its probably going to hapoen. So the dr hasn't said for sure yet what her best advice is. My main concern is being bed ridden with just a vaginal cerclage because my son has some disabilities and well bed rest isn't an option. With the tac there isn't any bed rest.

Not having bedrest would be a pro with the TAC. One potential downside is that it would require a c-section, but for many women that isn't an issue. Another option to ask about would be a TVCIC. Sort of a blend between a standard cerclage and the TAC. Might not need bedrest, doesn't require a c-section, and it is usually quite effective.

Certainly the TAC, while having some cons, the benefit of knowing it provides the best chance for a full-term birth, and particularly in your situation no bedrest, makes it appealing to many.
