Employee screening test. But does it include Cotinine too ?

An employer can test for nicotine. You can also ask your employer what they are testing for. They can test for a range of drugs, or specific ones.

If it is a hair drug test it most likely will not include nicotine. Ours doesn't. It looks like they are going to test for the NIDA 5 (main drugs of abuse) plus the extended opiates (pain killers).
Here is the list of drugs tested for along with cutoff levels.:
Cocaine/Metabolite 500pg/mg
Marijuana Metabolite 0.10pg/mg
Amphetamines 500pg/mg
Methamphetamines 500pg/mg
Opiates (Cod, Mor, Her) 200pg/mg
Phencyclidine 300pg/mg
Ecstacy (MDMA) 500/mg
Cutoff levels for the Extended Opiates are 200pg/mg include: Vicodin (Hydrocodone), Lorcet/Lortab (Hydrocodone), Oxycotin (Oxycodone), Percocet/Percodan (Oxycodone), Dilaudid (Hydromorphone)
