when you have extremely high blood pressure and have tried most medications what do you do?

Do You mean that your doctor has prescribed meds that do not reduce your high blood pressure? What is your blood pressure reading normally, Paraclete?
Normally this is your doctor's job to solve that problem; there are other contributing factors causing high blood pressure such as high cholesterol, or plaque clogging the arteries. There other meds used in conjunction with high blood pressure meds to reduce it accordingly.
If your present doctor is not helping you, get another one.
High blood pressure is called the 'silent killer'.

There are combinations of medications that work well together. It's trial and error to find the right combination for you. Proper diet and exercise are also important.

my blood pressure was controlled using mycardis 80 until about two years ago 140/80 we then entered a period where my blood pressure elevated various medications were tried without success including a visit to a specialist recently we again had it under control until I took a medication for gout and it went very elevated and is defying attempts to control it, it was 219/86 the other day. We are now at the point where we basically have run out of options as side affects preclude the use of certain medications. I'm uncertain about my doctor too as he got two vastly different readings in a recent visit to him, begging the question how often in the past did he get it wrong. We are speaking about a very experienced doctor here. I presently take a number of drugs to control blood pressure so the coctail isn't working and of course the doctor isn't open to anything but incremental change
