My ex wife made my daughter birth certificate without my name. How can I add my name on her birth certificate? My daughter was born in NY but moved to Florida a year ago. I'm still in NY.

Most likely you can't. A woman can put whatever name she wants on the birth certificate. On the other hand, if you were married at the time your daughter was born, you should at least be listed as the father.
You would have to go to court to petition for the name change. But without the mother's agreement, its unlikely a court will approve it.

We were married when my daughter was born but she didn't put my name on my daughter birth certificate, it is blank.
She doesn't want to add my name because she want to send my daughter to other country in December and without my name she can do whatever she wants without my authorization, right?
How can I change that situation? I already have the DNA.

Have you seen the birth certificate? Are you legally divorced? In NY, the husband is presumed to be the father. So I'm unsure how she managed to leave your name off. Not giving her your last name is a different matter. If you are legally divorced, you should have been given some level of custody. Again, making you the legal father.
The first thing I would do, is contact the State Dept and see what you can do to prevent a passport being issued for your daughter. I don't believe it will be as easy as that for her to leave the country without the father's permission. I would also make sure you register with New York State Office of Children and Family Services (OCFS) (http://www.ocfs.state.ny.us/main/publications/Pub5040text.asp), While this is primarily for unwed father's you need to make sure that somewhere you are listed as the legal father.
Finally, you need to contact a lawyer about this. There may be other things you can or should do to prevent your daughter from leaving the country.

If you are the biological father you would have to go for a DNA test and get your name on the birth certificate.

He already has the DNA result, and saying 'get your name on the birth certificate' is the question, not an answer.
