How to close an autosweep Multi option deposit scheme account?

I had account in SBI Pune. I had relocated 3 years back to chennai.
Now I want o close the autosweeped deposit accounts there in pune.
Becos I faced problems in filing income tax as bank had n'tgiven me any form 16 , even after my continuous requests.
