In doing this, does it mean that the biological parent still pays child support and any debt they run up?

I've seen it written both ways - what kind of debt?
Would this be court ordered?

Yes.. They told my son since he is out of state that they need this so they can take child to dr. sign them up for sports etc.

My son is not behind in child support, They also told him not to show up for court.

By debt I mean anything done without his permission, Medical etc.

I would not sign anything without having an Attorney look it over. Emergency medical care and often medical care in general are in different categories than other expenses.
Maybe if the child needs shoes the birth parents should be consulted. If the child is sick or injured, no, someone has to seek and pay for medical treatment.
That's a rather odd queston. What exactly are the circumstances? Parents seldom worry that there are going to be unnecessary medical expenses or don't want to pay for them.

Guardianship means the legal parents give authorization to other parties to provide legal care for their child. They still retain parenthood with all its rights and responsibilities.
They will continue to owe child support, however, since child support is paid to the custodial parent, and that parent would no longer have custody, a modification may be possible.
If the guardians incur medical bills on behalf of the child, he could be held responsible.

What exactly is the custody arrangement that the courts have approved? In most cases if both parents have legal custody then either parent cn sign guardianship over to a third party under certain conditions. What are they wanting him to sign? This sounds like a pathway to get his rights removed. Be very careful.

I have a couple of concerns -

Is this asking about a consent form - I have consent forms for my stepgrandchildren for similar purposes.
And I agree - it's Attorney time!
