My daughter is 8 months old and has my name first and her father's name last. I want to know how to go about dropping his last name. Short story, he left me when I was 6 months pregnant to be with a women who he had been cheating on me with. He was there when my daughter was born but since then has not been. He has not provided my daughter with any supplies she has needed since birth. He has not seen her in 2 months and the last time I heard from him was 5 weeks ago. Since my daughter was born in October her father and his girlfriend have gotten physical with each other and because of that he tried to kill himself by walking into traffic and was then put in a psychiatric ward of the hospital. He contacted me in April after I had not heard from him in a week, saying sorry he had been in jail, which if it's true is the second time he has been in jail since November 2011. He constantly lied to me about anything and everything. I do not trust him with my daughter and do not want her around him and his girlfriend together. I have not taken him for child support for fear he may get her. I know I may sound selfish but I am just trying to protect my daughter. We live in Pennsylvania. Thank you.
