Has he consulted with his doctor? There may be blood pressure or prostate problems or other things going on. The doctor can refer him to a specialist or sex therapist, depending.

Yeah he has been to the doctor who reluctantly prescribed Viagra but other than that gave him a clean bill of health

Did he have is prostrate checked, not done in routine check ups, ( major cause of erection issues)
Is there money issues ? Is there work problems.
Does he drink, use drugs ?

Also for men, once they have one or two problems, they worry and it only makes it worse.
Even if he is not hard, if he masturbates can he ejaculate?

He should. I had a issue some years ago, well first figured it was weight, and high blood pressure issue. I did the Viagra route, tried some of the others, several over the counter things. Turned out in my case, and I waited longer than I should, it was a prostate issue, I was lucky and got cured. So I do preach that a bit.
