My wife has been home from the hospital for about 3 days and all of a sudden she has a quarter sized lump under the skin directly beneath where she had an IV in her hand. How should we treat this?

Contact her doctor.

It sounds as though her IV site may have infiltrated. You can use moist heat to assist in decreasing the swelling. If there is and redness or heat she would need to see her doctor.
