You have not asked a question, but owing to the way you laid out her analysis I think you are asking how to control her creatinine levels. Any concerns about her medical make up should be discussed with her doctor.
Is she a diabetic? If so, these are critical decisions to be made for her care.

My question is what do these results mean ?

High creatinine and urea means increased protein leakage into urine and is a sign of chronic kidney injury. We can't diagnose on the internet, and this opinion is my own based on anyone having high creatinine and urea levels.
So, she is a diabetic, who has not been following any kind of regime? Discuss this with her doctor.
