My brother was adopted by a family that he and I both lived with when I was in foster care and now that I am 19 I want to know how I could reopen our case and adopt him as a part of my family...

No, you can't. Once an adoption is granted, they are his legal parents and no court is going to overturn that adoption unless you can prove abuse.

OK so if your adopted by a family is it mandatory that you take on their last name and things of that nature

No, but it's the usual procedure. It has nothing to do with the legality of the adoption. If the child is a minor, this is a decision that the adoptive parents make.

However, once he reaches 18, he can change his name to whatever he wants.

An adoption is not going to be changed. Nor should it, this family has cared for him all this time.
Is there a issue, is he being abused, is it unsafe ?
It is common for a child's name to be changed when adopted, but is not rquired.
