5 yrs ago, I was in a really bad place in my life, and I failed to immunizize my 5 yr old daughter. She just started kindergarten and they are asking for her shot record. What should I do? What kind of catch up schedule would that be?

You need to ask her Pediatrician. The school nurse very possibly can give you a schedule for her.
She's had no immunizations?

Go to her doctor, pediatrician or local health clinic and get the immunizations.

Yes your doctor can catch her up. Be sure your doctor understands to do it safely however. I have heard mothers say that their doctor caught their children up by giving 6 shots at a time every week. That is TOO MUCH. 4 at a time, every 2 weeks (if not a month depending on which shots) would be the max for me. But that is just my opinion.

Yes, you just take the child to the doctor or local health department,
I guess I will ask the question, if you failed to do this, is there other issues that causes you to neglect your child's health care
