I have more than one question but the most important one is what the hell is 'cavdi' deduction? Line 14 of the w2 form.

2nd question> 5 years on F1, and was on 'opt' in 2006 (all of 2006) and now on HI. What other tax forms should be filed along with the 1040 (I don't quality for 1040 NR do I?) ? Also if employeer has not taken out an SS or Medicare - does anything need to be done by me, am I suppose to pay those while on 'opt'?

Not sure what CAVDI means. What state are you in?

You are a resident alien. File Form 1040.

Inform the employer of your resident alien status.

CAVDI is probably an acronym for Disability Insurance.

CAVDI stands for California Voluntary Disability Insurance. It is an acronym for the private insurance some employers opt for rather than paying into State Disability Insurance.
