I notorized a signed letter for a man that wants to marry a woman from the Czech Republic. It was hand written stating who he is and where he lives. There is a lawyer working where I work and I asked her as she walked by if this way OK to notorize. She said yes... I am also used to only notorizing things at my jobs for the people I work with. No excuse but this man came back the next day with a form from the county clerk and written on there was NOTARY VIOLATION. Because I did not have that paragraph typed out about, "on this day of... came to me... and proved it... you know the paragraph... which was also on the paper as an example of what I needed to do. I showed the lawyer at my office and she said Yes, you should have had him fill out a form and so on... this poor man was told to do the hand written letter for the Czech Embassy and is really very confused. Now I am so sorry I tried to help him at all. Anyway, I typed up the correct form and he is now talking to the lawyer about his next step... I gave the form to her but she has not done anything with it yet. He proved he was who he was with a US Passport. Could I be in trouble?

Yes, you could lose your notary license. The purpose of notarization is to insure the signer is who they said they are. If you don't go through the proper process then you negate the value of the notarization.
