I dated my primary care physician. After we broke up he sent me 4 emails with personal attacks using the health information he knows to hurt me. In his emails he writes things like: "Nobody loves you, I don't care if you alive or not, I am dating a person who has no physical problems such as yours: Dx...., Dx....".
This is very depressing and stressful. I am not sure if he can hurt me like that just because he dated me before. What is your opinion?

Telling your problems to you doesn't violate your hippa interests. They are after all your problems. Now if he were to spread information about you to others then its different. But to you directly represents no violation.

On the other hand you might want to look at your states guidelines for doctor patient relationships. From that standpoint he may be crossing some boundries.

Not a HIPAA issue at all. You can choose to not read his e-mails, click on SPAM and do not open them.

Yes time to go to NO contact and stop reading any email, reading any text and do not answer any calls
