I'm curious as to why you would choose concrete blocks? If an exterior doorway, then best would be to do it in a way that matches the house.

As for labor charges, that will vary from contractor to contractor. Call 2-3 local masons or home improvement contractors and get quotes. They'll give you quotes for free... so just pick the one with the best price and reputation.
