Are you living with the child's mother ? Have you established paternity in any method, such as DNA tests ? Is the child's mother OK with adding you to the birth certificate

Both parents would need to sign and submit this (http://www.cdphe.state.co.us/certs/paternity_acknowl_2011July.pdf) form. If the other parent will not agree, you should contact the Colorado Department of Public Health and Environment. (http://www.cdphe.state.co.us/ic/infohom.html)

I do live with the mother and she is OK with adding me to the birth certificate. Also, right now he has a hyphenated name... {Names removed}". Will that form you told me to fill out change that as well?
