My 2004 Honda Accord VTEC AT sedan bought in the Philippines has a broken cable reel with part no. 77900-SDA-Y21.
Can I replace it with a cable reel with part no. 77900-SDA-A21 bought in the US?

Frankly, I don't see why not. That is likely a standard Honda part number and not limited to any particular market.
