Ok so my daughter is autistic, with several other imparing issues.

Her doctor suggested we apply for SSI. So we did, in November of 2010. We are still waiting and getting various pieces of paperwork to fill out and send back etc. we have a lawyer, I suppose I can call and ask him. But sometimes it is hard to get through and this is a fairly simple question.

The DES (dep. Economic security) sent me a teachers questionnaire. For her teacher to fill out and send back.

But we homeschool. Should I ignore this paper? Or fill it out myself as her teacher? OR would that be a bad idea, as I am her mother as well and it would be a conflict of interest?

Thanks for your time. If no one here knows I can call my daughters lawyer Monday morning

I'm only guessing that it would require the state or local worker who monitors the home schooling.

That was my thought, as its not traditional homeschooling, its called k12. It's a public school done at home and online. She has a teacher. But the problem I'm thinking will arise, is it is asking about behaviors in the classroom and her teacher, while she is wonderful and supportive and helpful, doesn't spend any classroom time with my daughter. So I'm concerned that she wouldn't know the answers to some of thse questions.

Given that it's all bureaucratic paperwork, I would guess that you could fill it out and she could sign her name to it, or you could fill it out together.
