A bank with a trust department can set up a trust easily, but the hard part is defining the terms of it's use. I would not try to start this all on your own from scratch or you will spend too much on lawyers and consultants. I would go to a university medical center and talk to people in the diagnostic lab. Expanding a current lab to have the latest technology would be the best use of your funds. Labs across the country are not at all equal in what they have. There's also the matter of hiring the best diagnosticians. You can be as involved in all the decision making as you want to be, but you may also need to compromise on what the people you chose to help set this up feel is needed.
