The mother of my child is causing me so much problems and destroying my life as much as I want to be with my child I can't deal with the depression the child's mother is giving me so what forms will I need to sign to give her up to her mother

There are none because you can't do it. Only a court can grant a Termination of Parental Rights (TPR) and courts are very reluctant to do so.

You can, however, walk away from your child and end contact with the child and its mother. There is nothing requiring you to be an active father if you don't want to be. But you will still be help responsible for child support.

Personally, I would prefer to fight for my child rather than abandon them. If you tell us more about your situation we may be able to suggest avenues you can pursue.
