View Full Version : Wrong putative father's name on birth certificate mother's mistake AR?

There were two possible fathers. Later found out (genetic testing) that the wrong person was named. Neither man married mom. Mom and bio father now want bio-father's name on certificate. How would I do this?

It's a relatively simple procedure - ADHS: Division of Public Health Services: Office of Vital Records: Changing the name of the child when the child is already named on the birth certificate (http://www.azdhs.gov/vitalrcd/amend_05.htm).

It is a crime to put the wrong name on the because but it is almost never prosecuted.
