I used OPT in 2009 about 10 months. But my employee withheld medicare and FICA in that period. Now I want to file a 843 form, but I am not sure what to file in line 6. should it be other -- w-2 or 1040?

If you were a non-resident alien (and only in that case you would not be liable for social security and Medicare) you should have filed a non-resident alien return on form 1040NR or 1040NR-EZ.
So you would enter on line 6: other -> 1040NR (or 1040NR-EZ)

In this case, TaxForAliens is wrong.

On Line #6, check off the box next to Form 941, which is the Quarterly Employment Tax form filed by your employer.

Additional, Line #3 should be "Employment".
