Hi I'm from northern Indiana I was wondering how I can legally get my fathers name on my birth certificate he passed away in 1993 and my mother will not do put it on for me. I have asked her. It is really important to me to do it. He was also born in Indiana but I'm not sure where at. Im not even sure where to start

Was he proven to be your father through DNA?

It's quite doubtful that you will be able to do it. You are, apparently, no longer a minor. And the man who you believe to have been your father is not in a position to object to such a change.

But where to start would be the Indiana Department of Health (http://www.in.gov/isdh/). Call them and ask.

This FAQ (http://www.in.gov/isdh/20243.htm#VitalFAQ9)suggests that it is probably even too late for your mother to change the BC, even if she wanted to:


"A paternity affidavit can be completed at the hospital within 72 hours of the birth or at the local health department in the jurisdiction where the child was born within the first 18 years of the child's life. "
