I am working on a case study in a situation where it was completely environmental. So would that only be axis 4? Or does something from either axis 1 or axis 2 need to accompany it?

Why do you think it's environmental only? Often more than one axis is used.

* Axis I – Symptom Disorders
* Axis II – Personality Disorders
* Axis III – General medical conditions
* Axis IV – Psychosocial/environmental problems
* Axis V – Global assessment of functioning (often referred to as GAF)

I think it is environmental because it is about a woman whose husband drove her to attempting suicide. He was controlling and very sexist. But it said 2 months prior to the attempt she had been seeking therapy for tiredness, loss of appetite and sleep disturbances. So I don't know if that was also because of the husband or something else.

What about Axis III? You don't know the source of her medical symptoms.
