I am 23 weeks pregnant with my second child (fourth pregnancy) I suffer from hyperemesis gravidarum and have been classified as high risk. My obgyn told me that I gained 7 lbs in a week then asked ME why. I told him with my son I gained lots of weight in short spurts and was monitored for protein in my urine. He just said that my swelling will get much worse, didn't examine me or do any tests only said he would see me in a month. When I asked to speak to the only other obgyn in town I was told that I'd have to switch obgyns just to get a second opinion. Is this serious? Should I demand testing, or?

You could just be following the same pattern as you did with your first. Sometimes there can be a sudden growth spurt. If your doctor was truly concerned, he would have likely ordered an ultrasound.
It is not uncommon to put on some weight a bit faster at this point in your pregnancy. Of course your doctor would want to look into it should you continue to have such a rapid weight gain.
Did you have the usual routine check for protein in your urine at the visit? Did he give a reason for why he believes the swelling will get much worse? Did he suggest ways to possibly help limit or even decrease any swelling? Where are you showing signs of swelling... feet, hands, legs, face, etc?
Do you still suffer from hyperemesis gravidarum? Are you taking anything for it? Sometimes once it is under control, or has subsided, women will put the weight on, and while it can look like a large amount at once, it balances out for what they likely had lost before.
You could ask about having an ultrasound done to ease your concern. The peace of mind would be worth it.
