Does the a.Income taxes part equal State Income Tax(box 17 of W2) + CA-SDI(state insurance disability) tax(box 14) + Any additional state tax paid in last year's state returns?
In other words, can you include any state taxes paid in the months of Jan-Apr 2010 in Lines 5-8 of the Itemized deductions worksheet?

You can include all state income tax payments that you made or that was withheld by your employer from your wages in calendar year 2010 - even if it was against your 2009 tax bill. The only requirement is that you actually paid it (or had it withheld by your employer ) in calendar year 2010. You can also include the CA-SUI tax that was withheld in 2010.

Also I have another question. I paid additional federal tax in April 2010 while filing my returns for the year 2009. Is there a way I could report that payment as a deduction? If so, where should I include that?

No - payments of federal tax are not deductible from your federal tax.
