Wife was fired at her employer do to in compliance of the doctor office nurse, refusing to complete FLMA forms correctly, even though she was compliant to all of her office visits. She have diabetes, gout and High blood pressure. We received an anonymous phone called stating that her rights was violated do to the RN... refuse to complete FMLA and she also called/text her husband about my wife was fired, which he works at the same place of my wife... the caller would not reveal themselves, so I don't know if it was some one from her husband job or someone from the doctor office which called and gave this information.

You need PROOF in order to file a complaint.

Why did the Doctor's Nurse refuse to complete the forms?

I don't know why she refused to file out the form. Also her blood pressure was high when she made her last doctor visit which was the reason they had her out on medical leave. All I know that the call we received stated that information and that someone also from her job called and asked her how did she get fired while she was out on medical leave.. which was suppose to be rumor around her job and that came from the nurses husband at my wife job...

You may very well have an action here but I think proving it is going to be difficult.

Was your wife actually fired? What reason was given?
