I would guess your son has an ear infection that is particularly bad in one ear. It hurts more at night because when he lays down, the pressure increases in his ears, causing pain.

Ear infections are very common in children your son's age. It is very important to take him to the doctor and get him on antibiotics if they do confirm it's an ear infection because it can impact his hearing if it's not treated. It is fairly common for small children to even get chronic ear infections so you need to watch for signs like being crabby, clumsy, not wanting to lie down, pulling, tugging or putting things in his ear, running into things (can effect balance) and being clumsy. Some kids get fevers with ear infections, too.

If a child gets chronic ear infections they often will put tiny tubes in both ear drums which allows any fluid build up to drain, and generally stops the infections. The children naturally grow out of the tendency to get the infection as they grow and the angle of the ear canal increases, improving natural drainage from the ear, and at that point, the tubes fall out on their own. You can't see or feel them in the ear - they are very small. This is only done though, if the infections are chronic. Otherwise a round of antibiotics does the trick most of the time.

I'd take him to the doctor today.
