I had a c-section on January 3rd. I had the normal bleeding after that stopped. About 1 week later I started bleeding again. My doc said it was probably my period. I thought that was odd since I was breastfeeding. As each day went by my bleeding progressed. When I was changing a pad every 30 minutes the doc asked me to come in and have a ultrasound done. I did so and everything was normal except the heavy bleeding. The doc said he was clueless as to why I was bleeding or where it was coming from. Then about 1 hour after seeing him the bleeding got heavier and it was just flowing out causing me to change a pad every 20-30 minutes. I called my doc and he asked me to come back in and repeat the ultrasound. I did so the next day and again everything normal, blood count good, no signs of where the blood is coming from. Then about an hour after I left the office, around 4:30pm the bleeding was so bad I was changing a pad every 10 minutes. I called my doc who said that a D & C needed to be done just in case there is a piece of placenta they just can't see there.

So I went to the ER and checked in for an emergency D & C. While waiting to have the procedure done, the bleeding increased to where I would sit on the toilet and blood would just pour out for up to 2 minutes. Straight. The D & C was performed, regular bleeding post D & C occurred and then stopped. I was happy, thought yeah problem fixed. I called to check results on what they scraped and they said no placenta or abnormal tissue found. Then exactly 2 weeks after having the D & C the bleeding came back. I went back to the doctor, had another NORMAL ultrasound, and still no answer. So my doc said we could try two things, one a high dose of estrogen and if that doesn't work then a hysterectomy. So we tried the high dose of estrogen, which upset me because I could not breastfeed during this time. The bleeding stopped and the doc said after the fifth day of pills for me to stop taking them, get back on the mini pill, and then 24 hours later breastfeed again. So this worked and the doc and I were both excited.

Well now its been 3 days since the last dose of estrogen and 5 days since I last bled. Well last night the bleeding started up again and within 1 hour was full blown changing a pad every hour. I went and saw my doc this morning. He did a pelvic and said well for it being a hormonal problem that I had a lot of blood coming out. I told him that is how it has been since the beginning. He suggested we start back on the estrogen with a high dose and then decrease to one pill a day once the bleeding has stopped. I am to keep an eye on the bleeding and if over the next three days if it does not decrease to call him. If it does then lets continue with one pill a day for two months.

My concern is could we be missing something? Is there something he is just not seeing? He has consulted with 2 other docs in his group about me and they all scratch their heads. The bleeding is bright red with no clots. Can anyone suggest anything?

Have they done an actual blood test to check your hormone levels? Since the addition of estrogen (which is often the first step to try) appears to make a difference, it may simply be due to a hormone imbalance. Were you taking the mini-pill already when all of this started? The mini-pill itself may be part of the problem for you. A combination pill may be effective once the bleeding is back under control (there are ways to keep milk production up if the mini-pill alone was not an option).

What was the dose of estrogen you were on? Breastfeeding is usually still an option.

If you were to continue to have problems, even with the estrogen, you would have a couple more options to try before going to a hysterectomy.

They have not done blood test for hormone levels. I was not taking the mini-pill when the second round of bleeding started. I actually started them a day after and it did not make a difference. So right now I am no longer on the mini-pill and I am now taking Loestrin FE 24. I am instructed to take 1 pill QID until the bleeding stops, then 1 pill TID for 2 days, 1 pill BID for 2 days, and then 1 pill daily for two months. The doc said no breastfeeding until I am taking only 1 pill BID. The estrogen did not affect my milk production the first time, so I am hoping it continues this way.
