How do I put my dad on my birth certificate? My mom never put him on and I was never given his last name. I recently met him in 2008 and he was never in my life until I found him after I turned 18? What do I need to do in order to get him on my birth certificate?

You have two options in your case:
1.Your father to sign Paternity acknowledgment/there is no time limit if a person does not have presumed father/
2. Through judicial proceeding./the procedure varies from state to state/

Will he need to do a DNA test to prove that he is my father? I live in virginia with him so how will it work?

It's up to him.

You may get Voluntary Acknowledgment of Paternity form from your local Department of Health.

You should have the Voluntary Acknowledgment of Paternity form notarized (witnessed).

And with no disrespect, it may be best to get one , just to be 100 percent sure.

And I will add it is great that you were able to connect with him after all of these years.

Well I know 100% sure he is my father my mom told that he was and thanks I'm glad too to have him in my life.

I have one more question once I do get my dad on my birth certificate will my last name change? Ive never been in this kind of situation before so I don't know how it works.
