Not sure where you live, but a name change is done though the courts, not though a school.

The school is support to use the child's legal name, but can and do use nick names

So what does she say when you ask ? Have you informed the school that legally the children's name are >>>>>.

If you need to, you will have to take her back to court to clear it up

She tells me our child is happy with this.
I have not got to the stage of contacting the school as yet, as i want to find out what legal rights i have first. So i know what to say or do.

What rights you have depends on what rights you have in the child custody agreement you got

This recently happened to myself. My son went into his second year of schooling and I was only made aware when his teacher had written his name on his book... my name was missing! I went into school the next day and asked the school registrar for my sons details. She showed me that he was registered under his full name which is on his birth certificate but his mother had changed his "known as" to her name, taking mines off. She only done this prior to his second year of schooling. I made the registrar add my name. Update my details and made her aware that any literature regarding my son had to be sent to me as well as his mum. I also informed her that if they changed his name in future without my consent I would take the school to court. Of course his mum denied ever doing this but sure are'nt all women angels!
