Get a second opinion. Talk to a new doctor about the issue.
Sometimes doctors get too comfratable. Using the frog analogy, if you put the frog in boiling water, it will jump right out. But if you put a frog in cold water and slowly head it up, it will just sit there and boil to death.
It sounds like the doc is so comfratable he doesn't notice the red flags.

What does he eat in a day's time?
Does he drink at least 8 glasses of water per day?

My little sister has the same problem, unfortunately doctors never recommend anything more that laxatives. However soft foods like yogurt and pudding might help but I'm sure you know this already.

The best thing you can do is get MiraLax. It's wonderful and is not addictive.
It draws fluids from the tissues in the body to the colon to help soften the stool and help the child have a proper bowel movement.
I have been using this on my son for 6 months now, and swear by it. You may also have to do some bowel retraining as well.
