Word 2003. Creating a template. I want to lock in the underlines so that one can type info on top of the underlines without deleting the underlines or moving the line. How do I do it?

Example: The template might have the following lines:

Name: _____________________________

Date: _____________________________

Service: _____________________________

One opens the blank Word doc based on the template and then scrolls to the line to type the information. I need the blank underlines to be locked in place so that one can simply type the info there keeping the underline in tact. How do I set up the template this way? Thank you very much. I hope you've found an answer by now, but just in case:

I use tables.
Suppose you want:

Name: ___________ Date: ______________
Address: _________ Phone: _____________

You create a 4-column table. Put Name: and Date: and Address: and Phone: in the 1st and 3rd cells of row one and two. Click in the other cells and hit the bottom underline border button.
