View Full Version : Name change in birth certificate?

My daughter was born on 21 jan 2010 . Her name in her birth certificate needs correction i.e.. incorrect:ameena correct : aamena . Can anyone tell me what is the process and what documents are needed ? Since she is just 7 months old I do not have any proofs of her correct name

If you are looking to correct a misspelling of the first name, just go to the agency that issues birth certificates and ask for a correction. If you don't know what agency to go to (and it varies by locality) ask at the hospital.

First of all, did you receive the Birth Certificate in the mail? If so, the hospital where she was born should have the record of birth on file. Either call or go to the hospital and see what they submitted to the Health Dept. as far as the spelling is concerned. If they submitted it correctly, then the Department of Records (Health ans Human Services) is at fault and they will have to send for proof and documentation to the hospital of her birth for verification of spelling for the name. If you signed the paper that stated her name was correct, and in fact it wasn't, then it will be on you to put in for a name change to the court system in your state.

Is this a first name or a last name? If a last name then you will have to provide proof of the change to make it happen and repair a mistake.
