Would anyone be so kind as to post an image that shows the connections of a correctly connected bathtub and/or shower stall (with dap out box etc.) before the cement has been poured?

Hey Joe,
In my area wet vents are allowed,(see image). This will allow just a single vent for the group. Check with your Building Department to see if it's allowed, Good luck, Tom

Thanks Tom... I was looking more for an idea of how the connections from the back of the tub look in their proper places...

i.e. what the "dap out" box should look like, where the trap is located in relation to the floor and tub, how accessible the trap and drain/overflow should be

Also, am I correct in my understanding that once everything is connected, you back fill it the boxed out area with cement?

If you know the center of your tub drain (measurement) you can put your trap in the ground before you pour. You can build a box from wood materials or even use a tupperware type container as your box. I usually make mine about 12" square. Have a riser pipe coming off your trap at one end of the box, maybe with an inch to the end, centered side to side with your trap under the rest of the box. This way you have a little wiggle room when hooking up your tub. And unless you are worried about radon gas I wouldn't be concerned with filling the box with concrete after you are hooked up. Sand or pea gravel is fine.
