What should I be eating on a daily basis to get all the nutrients I need?

To get all the nutrients you need, you need to eat from the basic food groups.

Fresh whole food. Base your diet on oily fish, eggs, fresh meat and chicken, lots and lots of green veg, some dairy, some fruit, some nuts, some wholegrains. Reduce the amount of sugar and processed food.
Eat round the outside of the supermarket, so you fill your basket with veg, fruit, fish, meat, milk, cheese (cottage if dieting), oatmeal, wholegrains, etc. Avoid the center aisles which are full of packet and processed foods.
Eat fresh. Avoid things with "Eat by" dates that are years in the future.

You can download free software to track your nutrition. I recommend Cronometer. I have always eaten as Eileen G suggests, but after tracking my meals for about a month I found that I was consistently low in some key nutrients. I have made small adjustments to my eating habits to correct that, and find that I have been even healthier and feel better than ever.
